By submitting this form, I am giving Degree Fitness Seaforth my cancellation notice.

I understand that my notice must be given 30 days before my renewal date.
I understand that if my cancellation was given with less than 30 days prior to my renewal date, I will be charged for the following month.
Should I choose to rejoin Degree Fitness Seaforth after my cancellation is finalized, I understand that I will have to pay the current membership rates.
I understand if I have been away from the gym for longer than 12 weeks, that when I choose to re-join Degree I will be required to take some refresher sessions based on the Head Coach's recommendations.
All new members, or rejoining members will be subject to class availability and room in the gym.
I understand that I wil be removed from the Degree Member Facebook Group and ZenPlanner Member account will become inactive.
